I am a qualified eligible investor because I have an individual net worth, or my spouse and I have a combined net worth, in excess of US$1,000,000. For the purposes of this questionnaire, "net worth" means the excess of total assets at fair market value, including home, home furnishings and automobiles, over total liabilities.

I am an accredited investor because I had individual income (exclusive of any income attributable to my spouse) of more than US$200,000 for the past two years or joint income with any spouse in excess of US$300,000 in each of those years and I reasonably expect to reach the same income level in the current year.

I am a corporation, partnership, organization, trust, or other entity not formed solely for the specific purpose of constituting an eligible participant, which has a net worth of $5,000,000 and enters into a section 4(c) contract market transaction in connection with the conduct of its business; or which has a net worth of $5,000,000 and enters into section 4(c) contract market transaction to manage the risk of an asset or liability owned or incurred in the conduct of its business or reasonably likely to be owned or incurred in the conduct of its business.
